Request for Discontinuation of Faculty Appointment
Discontinuations are initiated by the department chair using the form linked below and are processed through Faculty Affairs. Faculty members do not complete or sign this form.

Faculty appointments may be discontinued for reasons such as resignation, retirement, relocation, or failure to complete the Maintenance of Appointment process or annual academic review. A conversation between the department chair and the faculty member is strongly recommended prior to discontinuation to help avoid potential misunderstandings.
Please note that separation from the Carilion Clinic system does not automatically require faculty discontinuation if the faculty member still intends to participate in the education of learners at VTCSOM, with the approval of their department chair.
Individuals whose faculty appointments have been discontinued may be reinstated if their time away has been three years or less, the department chair confirms the individual continues to meet the criteria for the reinstated rank, and the Dean of the School of Medicine approves the reinstatement.
A faculty member whose VTCSOM appointment was discontinued for not completing the Maintenance of Appointment process may apply once for reappointment. The individual is not guaranteed the same rank held prior to the lapse as this would be treated as a new appointment.
